SILVER PLAN $165 a year

Includes: two free prophylaxis cleanings, one set of bitewing X-rays, two free exams and two fluoride treatments per year, along with 10% off treatment we do in our office. (Does not transfer to referral offices.)

GOLD PLAN $275 a year

Includes: three free prophylaxis cleanings, one set of bitewing X-rays, two free exams and three fluoride treatments per year, along with 20% off treatment we do in our office. (Does not transfer to referral offices.)

PLATINUM PLAN $385 a year

Includes: four free prophylaxis cleanings, one set of bitewing X-rays, two free exams and four fluoride treatments per year, along with 30% off treatment we do in our office. (Does not transfer to referral offices.)
